Since this suggestion format of yours is quite obese, will make my own which is ACTUALLY understandable for the amount of suggestions i will suggest.

Since this is a ''realistic'' roleplay server. remove the /tell command, it isnt realistic. you should have players buy phones to be able to text them, (/phone add (number) (name) | /call (name) (message). Every player should have a /pn command which lists their OWN UNIQUE phone number.
#2 PROPER fighting mechanics
There should be a rule for RDM (random death match), meaning that a person cannot attack another person for NO VALID reason
keep in mind that, GANG related things should bypass the RDM rule to an extent. example: if a rival gang has for example hurt YOUR gang member, you may retaliate with whatever you got, killing allowed

ROLLING SYSTEM
player 1: /me *does a fighting move*

FROM BEHIND ATTACKS
Rolling should not be neccessary in this case, as you cant do anything when someone takes you from behind.
4-6 hits to the head neccessary until a K.O move can be performed by someone else. for a K.O, when a user is K.O They must at least be K.O for 5 minutes IRL time.
EDITTING /ME COMMAND
Make /me only show up in a specified range (preferrably a shorter range than speech.) (blocks). honestly, just do it.
/SIT AND /LAY
/sit and lay can both be used to make the fighting mechanic more ''realistic'' And both for cosmetic use, and general use.
For example, if a user makes a dropkick move, you would fall down. in this case, you would do /lay UNTIL the user being knocked down has done a /ME describing that they have stood back up, or rolled to the side, or whatever.
TEXTURE PACK
this might be a more difficult job, but its certainly worth it if you want the server to look more real.
for example: a katana texture, gun texture, cosmetic textures, road textures, billboards, block textures, PHONE TEXTURES. etc.
***METAGAMING***
metagaming rule should prevent users from saying locations of another player, object, place in GLOBAL chat.
GLOBAL chat could be a /OOC (out of character) command with a cooldown of 100 seconds, preventing the clogging of the proximity chat. /LOOC can be used for LOCAL out of character, 5 blocks extended reach than the normal proximity chat, with no cooldown.
