- Why do you want to be EOTM: I want to be the employee of the month for June because it would be my first ever EOTM award for doctors. I have been attending every patient I can possibly attend. I have a DEAA (drop everything and attend) attitude.
- What can you bring to the EOTM board: I want to get the ball rolling for the EOTM awards, as I think it would be a great incentive to doctors

I apologize for the misconception. I'm not sure where you got this template from, but this award isn't something that you apply for. At the moment, the only applicable position in the DoH is the Hospital Secretary role. I'm going to have to deny this post since it is under the applications thread. However, I'd like to make it clear, once again, that this isn't something that you apply for nor should this template exist.
